Experiencia de manejo endovascular de aneurismas con coils en hospital de referencia de Perú: reto terapéutico

Introducción: los aneurismas cerebrales no rotos (ACNR) tienen una incidencia del 5-10 %, y su rotura causa hasta el 98 % de las hemorragias subaracnoideas espontáneas, con alta morbimortalidad. Los avances endovasculares permiten distintas técnicas de embolización buscando mejor oclusión y menor recanalización.
Objetivos: evaluar la eficacia y los resultados clínico-angiográficos de la embolización simple, la embolización asistida con balón y la embolización asistida con stent en pacientes con aneurismas cerebrales tratados en nuestra institución.
Material y método: estudio descriptivo y retrospectivo de 61 pacientes tratados mediante embolización endovascular. Se analizaron variables clínicas, angiográficas y funcionales con las escalas de Raymond-Roy y Rankin modificada.
Resultados: predominó el sexo femenino (73.7 %) con edad media de 56.1 años. La localización más frecuente fue la arteria comunicante posterior (27.8 %). Se realizaron embolizaciones simples con coils (29.5%), asistida por balón (39.3 %) y por stent (31.1 %). La recanalización fue mayor con el uso de coils solamente (41.1 %) frente a balón (4.1 %) y stent (5.2 %) (p <0.01). La mortalidad global fue de 3.2 % y el 81.9 % de los pacientes presentó un resultado funcional favorable (Rankin 0-2).
Conclusión: las técnicas asistidas por balón y stent mostraron mejor estabilidad angiográfica, menor recanalización y baja morbimortalidad frente a la embolización con coils solamente. Se requiere optimizar el tiempo ictus-coils y descentralizar los centros especializados para mejorar los resultados funcionales y de sobrevida en pacientes con aneurismas cerebrales.
